Een penispomp gebruikt een vacuüm (onderdruk) in de cilinder om de penis met bloed te vullen en in erectie te brengen - zelfs in het geval van erectiestoornissen. De penis kan groter worden en neemt ook in omvang toe. Met voldoende training en tijd kunnen ook verbeteringen op de lange termijn worden bereikt.
